Where Does a Pass Mark Come From?

A pass mark is a judgement made by a panel of people through a documented procedure, and it is not a percentage of the questions. A raw percentage cannot be the standard because question difficulty varies, so the same score means different things on two forms of the same exam. How a panel defines the barely competent candidate as a person and then judges every question against them.
